The Support at Home program is an initiative funded by the Commonwealth Government, designed to provide ongoing support for older Australians who wish to continue living independently at home.

The Support at Home program introduces eight levels of ongoing support with annual funding ranging from approximately $11,000 to $78,000. Funding is provided quarterly, with a portion of unspent funds carried over to the next quarter.

The eight levels of Home Care Packages in Australia are:

Short-term Pathways and Additional Schemes

Restorative Care Pathway: A 12-week program (extendable) offering around $6,000 for individuals recovering from illness or injury.
End-of-Life Pathway: A 12-week program offering approximately $25,000 for people wishing to remain at home.
Assistive Technology and Home Modifications (AT-HM) Scheme: Provides separate capped funding for equipment and home modifications.
Assistive Technology Funding Tiers: Low (up to $500), Medium (up to $2,000), High (up to $15,000, or more with evidence).
Home Modifications: Three tiers available, each with different funding caps and time limits for use.

Participant Contributions and Key Details

Participant contributions vary by service type and income.

Clinical supports are fully government-funded.

Independence and everyday living supports require co-contributions depending on financial situation.

Lifetime cap of $130,000 (indexed) applies to non-clinical care contributions.

Existing Home Care Package recipients will be transitioned to equivalent Support at Home levels and will not be worse off.

Staying In Place — an award-winning model of aged care that empowers older Australians to remain living independently in their own homes for longer, while maintaining dignity, choice, and community connection.
Originally developed in Pingelly, WA, the Staying In Place model has transformed aged care delivery across 50+ communities throughout Western Australia and beyond. Following its success in regional areas, it is now expanding across the Perth metropolitan area, including Wembley Downs, Mandurah, and Kingsley.

How Home Support Perth Works

Home Support Perth coordinates Home Care Packages in partnership with HomeMade, an approved aged care provider, using the Staying In Place model to give clients greater control and local connection:
  • Local community members provide services as part of each client’s care plan — not just provider-employed staff.
  • Clients remain registered with HomeMade while Home Support Perth coordinates and connects them with trusted support workers.
  • Services are arranged through the Mable platform, where all workers are fully verified (police check, insurance, ID, references).
  • Available supports include cleaning, transport, social support, personal care, nursing, and allied health.

  • Clients choose who provides their care and when.
